Individualized Service Packages

Heart Disease
• Assistance with activities of daily living
• Heart-healthy meal preparation
• Doctor or Physical Therapist directed exercise assistance
• Maintain record of weight for the patient and physician
• Caregiver trained on heart disease: how to recognize, respond and report changes
Diabetes
• Assistance with activities of daily living
• Diabetes-specific diet and meal preparation
• Doctor or Physical Therapist directed exercise assistance
• Maintain daily records of blood sugar for the patient and physician
• Caregiver trained on diabetic care: recognizing symptoms of high and low blood sugar, attend to foot and skin care, and actions to be taken.
Stroke
• Assistance with activities of daily living
• Personal care
• Meal preparation and feeding
• Doctor or Physical Therapist directed exercise assistance
• Maximum patient transfer assistance; transfer with mechanical lift if required
• Caregiver trained on stroke care and special transfer techniques
• Blood pressure monitoring as directed
